These pages contain helpful information for healthcare and social services professionals who wish to refer a client family for Home-Start serves. Families may also refer themselves.
Families in need of Home-Start services are usually referred to us by health-care or social services professionals with the full knowledge and agreement of the family in question. Occasionally families will self refer.
The downloads on this page give sufficient information for the referrer or the family to understand the kind of service we offer, along with a Referral Form.

The Referrers Guide is designed as an introduction for the health-care or social services professionals who normally refer families in need to Home-Start.
The Notes for Referrers expands on this guide to give clear guidelines on how to refer a family for our services.
The Referral Form is to be completed by the Referrer, with the full consent of the family in question, and acts as the first stage in our application process. It should be signed by both the family and referrer.
Similarly, the Self Referral form is for families who feel the need for help and wish to engage with Home-Start to explore whether our services are appropriate for their situation.
